The Archdiocese of Chicago is pleased to announce that for plan year 2010-2011 Ceridian Benefits Corporation will be administering our Flexible Spending Plan. A Flexible Spending Account (FSA) is a plan in which you elect to put pre-tax dollars to cover the cost of certain out-of-pocket health care expenses for you, your spouse and your dependents. You decide how much money to put into an account during the enrollment period. Then, when you incur an eligible expense, you receive tax-free reimbursements from the FSA.  To enroll now, go to www.ceridian-benefits.com , and follow the instructions under “First Time Login Click Here” to register and enroll! As we conclude the current plan year, please note the following important items:

  • The administrator for claim incurred through June 30, 2010 will continue to be Employee Benefits Corporation (EBC).

  • Claims may be filed with EBC through September 30 for expenses incurred through June 30, 2010. The EBC Reimbursement form is availabe here.

  • The Benny Card will be shut off effective July 1, 2010.

For any questions regarding claims for the last plan year, please contact EBC at 1-800-346-2126. You may also access your account at www.ebcflex.com.

Flexible Spending At a Glance

The Archdiocese offers two types of FSAs:

Health Care FSA
This account covers medical, dental, vision care and other qualified costs. Eligible costs include plan deductibles, coinsurance, co-payments and expenses that insurance may not cover such as prescription and over-the-counter drugs, or orthodontia.
 
Dependent Care FSA
This account covers expenses you incur for dependent care while you are at work. Eligible expenses include daycare, nursery school and day camp for children, as well as services for older dependents that cannot care for themselves.

Money that is placed into the account falls under the "use it or lose it" rule of Flexible Spending Accounts. If you have any money left in your account at the plan year end, you forfeit that money. How do I Enroll?

All employees who were hired before May 1st, 2010 can enroll through Ceridian's Benefits website. Simply follow the link below to

All new hires to the Archdiocese interested in the flexible spending account should fill out the enrollment form and turn it in to their local benefits administrator. If you are not a new hire, you may enroll via the Ceridian website. If you use the paper enrollment form and you were hired before May 1st, 2010 it could result in a delay in initiating your flexible spending account for the next plan year. Using Your Flexible Spending Funds

Money from your flexible spending account can be used to pay for a variety of medical products and services. A list of these eligible expenses can be found if you follow the link below. Please note that not all expenses are shown, this list simply includes the most common expenses:

When paying for your eligible expenses, there are two different pay options you can choose from:

Option 1: The Ceridian Benefits Card

The Ceridian Benefits Card works just like a debit card. It automatically debits your FSA account when you use the card to pay for eligible health care expenses. Option 2: Claim Form

For dependent care expenses and for stores/offices that do not accept the Ceridian Benefits Card, you must fill out a claim form to be reimbursed for the approved expenses. The form is easy to use and is available on Cerdian's Benefits Website. Simply fill out the form and submit it to Ceridian. Depending on how you have chosen to be reimbursed, the money will either be directly deposited into your band account, or a check will arrive in the mail.
