The Archdiocese of Chicago offers you vision benefits to help pay for routine eye exams, glasses, or contact lenses for you and your family. All full-time and part time benefits eligilbe employees can take part in the vision plan.

Part-time employees must work an average of 26 hours per week to be considered benefit eligible. You coverage will take effect on the month following your first complete month of service for the Archdiocese. For example, if you are hired on June 15th, your first complete month of service would occur in July. Therefore, on August 1st your benefits will begin. You may also cover your spouse and children up to the age of 23.

 Current Vision Plans

 

VSP PPO Plan: The Vision PPO plan is a part of the Medical PPO plan. When you select the PPO medical plan, the VSP vision plan benefits become available to you. No contributions are requried for the vision plans through the Archdiocese.

 

Davis Vision HMO Plan: The Davis Vision HMO plan is a part of the Medical HMO Illinois and Blue Advantage plans. When you select either HMO medical plan, the Davis Vision plan benefits become available to you. No contributions are requred for the vision plans through the Archdiocese.
