The documents below are required to be completed by new employees hired by the Archdiocese, or provided to them for their information, as noted below.

Please note that we are legally required to provide certain documentation as indicated below.

I-9 Employment Eligibility Verification - Electronic, FillableThe full form includes instructions and the portion to be completed and signed. Form I-9 MUST be signed by the employee no later than the first day of work, and must be fully completed by the employer within 72 hours of beginning of employment. Download
