New Hire Required Documentation

New Hire Process - Updated September 2017

All new staff hired at parishes and schools must complete several documents at the time of hire, and must be provided copies of other documents.  All forms and documents, with brief descriptions of each, are available at the links below

 

The forms to be completed are as follows:

  • Employment Application

  • Archdiocese of Chicago Code of Conduct

  • Employee Handbook Acknowledgement of Receipt

  • I-9 Employment Eligibility Verification

  • Personal Data Form

  • Federal and Illinois Form W-4 for payroll setup

 

The documents which must be provided to all employees are as follows:

  • Employee Handbook

  • Prudential 403(b) Plan Auto Enrollment Contribution Agreement (Benefit Eligible Staff Only)

  • Health Insurance Marketplace Coverage Notice

Health Insurance Marketplace Coverage NoticeAll employees, regardless of benefit eligibility, must receive this notice upon hire.Download
I-9 Employment Eligibility Verification - Electronic, FillableThe full form includes instructions and the portion to be completed and signed. Form I-9 MUST be signed by the employee no later than the first day of work, and must be fully completed by the employer within 72 hours of beginning of employment. Download
